Terms and Conditions for the Immersion TherapyTM Service

Determined2 DOES NOT OFFER lessons in SCUBA diving. We do not teach or certify open water diving. Immersion Therapy™ is not SCUBA diving.
Clearances and Insurances
All Determined2 staff have received and passed a full DCSI background checks including Aged Care Sector Employment, Child Related Employment, Disability Services Employment, General Employment Probity, Vulnerable Person Related Employment.
Determined2 holds all relative insurances for the services it delivers. A copy of our certificates of currency can be requested at email@example.com
Fitness to Participate
There are no physical restrictions to accessing Immersion TherapyTM. All clients will undergo a medical assessment with an approved physician before any full body Immersion sessions take place. The assessment is done in line with the Immersion TherapyTM Key Medical Standards. You must have first attended a Meet and Greet session prior to receiving assessment.
Determined2 will provide you with a list of approved physicians at the Meet and Greet session and we will also make an initial assessment classification about your service suitability.
The certificate received as part of the Immersion TherapyTM assessment is not a fitness to dive certificate. The document certifies the Applicant as medically fit to participate in Immersion TherapyTM. The certificate is not valid for any other purpose. For the avoidance of doubt, the certificate DOES NOT certify fitness to participate in Open Water Diving or in any use of SCUBA equipment outside of the authorised application of Immersion Therapy. The certificate is to be used in conjunction with the approved Policies and Procedures of Immersion Therapy and the applicant must comply with those Policies and Procedures as well as the instructions of authorised Immersion Therapy providers.
Participant to Staff Ratios
The Immersion TherapyTM service is delivered 1 to 1 and in some cases more than one staff member may be required to safely deliver the service. All staff to client ratios are decided in line with the Immersion TherapyTM guidelines and the client’s classification.
All staff members have been specially trained under the Immersion TherapyTM training standards and guidelines.
The use of SCUBA equipment can be unsafe and very dangerous for younger children. In line with medical guidelines, we do not offer the use of SCUBA equipment to children under the age of 10. We can include children under 10 in participating without the equipment, for example on snorkel or just swimming, however, they must have some level of competency in water as we do not teach swimming lessons.

Your Information Release
I give permission for my personal information to be shared and discussed by Determined2 staff members, medical staff, claims agents, service planners and any other relevant service providers where necessary.
Media Release I expressly agree to the waiver and forfeiture of any intellectual, commercial or other property rights arising from photographs, narratives and other material whatsoever and howsoever used or produced by me or by Determined2 in the provision and promotion of its services.
Understanding Immersion Therapy I understand that Determined2 PTY LTD, any Medical Support or PADI (Professional Association of Diving Instructors) professional involved with Immersion Therapy does not teach, certify or imply that OPEN WATER DIVING is either recommended or supported as part of the Immersion Therapy program or for me personally. I understand that participation in Immersion Therapy does not teach, certify or imply OPEN WATER DIVING and does not allow me to SCUBA dive or use equipment outside of the Immersion Therapy program. I understand any medical clearances given to me is only for use within Immersion Therapy, and agree to follow guidelines set out by the medical clearance, Determined2 PTY LTD and any Training staff within Determined2. If you decide that you would like to pursue recreational open water SCUBA diving, you will need to source independent advice, complete the appropriate medical clearances and undergo any professional training within the set guidelines of PADI. I understand that I am not Medically cleared or certified to undertake any compressed gas (SCUBA) activities outside of the Immersion Therapy program and agree to all of the statements above.
Immersion Therapy Statement of Risk & Liability (Non-Agency) Acknowledgement I understand and agree that PADI (Professional Association of Diving Instructors) Members ("Members"), including Determined2 PTY LTD any individual PADI Instructors and Divemasters associated with the programme in which I am participating, are licensed to use various PADI Trademarks and to conduct PADI training, but are not agents, employees or franchisees of PADI EMEA Ltd., PADI Americas, Inc., or its parent, subsidiary and affiliated corporations ("PADI"). I further understand that Member business activities are independent and are neither owned nor operated by PADI. While PADI establishes the standards for PADI diver training programs, it is not responsible for, nor does it have the right, to control the operation of the Members' business activities and the day-to-day conduct of PADI programs and supervision of divers by the Members or their associated staff. Statement of Risk and Liability is a statement in which you are informed of the risks of Immersion Therapy. The statement also sets out the circumstances in which you participate in the Immersion Therapy programme at your own risk. Your signature on this statement is required as proof that you have received and read this statement. It is important that you read the contents of this statement before signing it. If you do not understand anything contained in this statement, please discuss it with your instructor. If you are a minor, this form must also be signed by a parent or guardian. Warning - Immersion Therapy has inherent risks which may result in serious injury or death. Breathing compressed air involves certain inherent risks; decompression sickness, embolism or other hyperbaric injury can occur that require treatment in a recompression chamber. Immersion Therapy can be a physically strenuous activity and you may be exerting yourself during this programme. You must advise truthfully and fully inform any medical professional, program professionals and the facility through which this programme is offered of your medical history. I understand that the Immersion Therapy program has been developed and is used by Determined2 PTY LTD and not PADI.
Acceptance of Risk I understand and agree that neither the dive professionals conducting this program, nor the facility through which this programme is conducted, Determined2 PTY LTD nor PADI EMEA Ltd., nor PADI Americas, Inc. nor their affiliate or subsidiary corporations, nor any of their respective employees, officers, agents, contractors or assigns accept any responsibility for any death, injury or other loss suffered by me to the extent that it results from my own conduct or any matter or condition under my control that amounts to my own contributory negligence. In the absence of any negligence or other breach of duty by the dive professionals conducting this programme, the facility through which this programme is offered, my participation in this Immersion Therapy program is entirely at my own risk. I HAVE FULLY INFORMED MYSELF OF THE CONTENTS OF THIS NON-AGENCY DISCLOSURE AND ACKNOWLEDGMENT AGREEMENT AND STATEMENT OF RISK AND LIABILITY BY READING BOTH BEFORE SIGNING THESE STATEMENTS
