Introduction

The Give-It-A-Go Fun-Raiser™ was designed and developed as a fundraising service to assist Disability Recreation and Sports SA in raising funds to cover cost associated with providing Immersion TherapyTM services to their non-funded non-compensable members.

Determined2 has no income associated with delivery of this service as 100% of the funds raised is directed to the DRSSA Immersion TherapyTM levy account, any running costs associated with the Give-It-A-Go Fun-Raiser™ is bared by Determined2, in no way is this service a replacement or variation of Immersion Therapy™ which has been specially designed to work with people living with a disability, injury or medical condition in a safe and appropriate manner within the Immersion TherapyTM guidelines.

We only offer this service as a way in which we can help support PWD in our community! It’s our gift to them.

 

Fitness to participate

Before you attend your session, you must have completed the self-assessment form available via the website HERE you will not be able to participate if you are unable to meet the medical requirements in line with Australian regulations and medical guidelines of “fitness to dive”, you will be expected to be in good health with no assistance required to attend the session, you must be comfortable in water and be able to both independently swim and support yourself in and out of the water.

If you are unable to meet these restrictions in any way we would highly recommend you apply for the Immersion Therapy instead.

 

Participant Numbers

While the Immersion TherapyTM service is delivered 1 to 1 the Give-It-A-Go FUN-RaiserTM is delivered at a staff to client ratio of 1 to 4 in the water, therefore any independent sessions must have minimum numbers of four fully registered and booked clients to go ahead, the only exception to this is a family or friend wanting to participate in someone’s Immersion Therapy session or where existing booking times in the online calendar exist, please refer to the online booking page https://directory.bookedin.com/determined2.

 

Age Limitations

The use of SCUBA equipment can be unsafe and very dangerous for younger children, in line with medical guidelines we do not offer use of the equipment to children under the age of 10, for further information you should seek professional advice from your local hyperbaric specialist, you can find your local specialist by visiting the SPUMS website https://www.spums.org.au/

We can include children under 10 in participating without the equipment for example on snorkel or just swimming, they must have a high level of competency in water, we do not teach swimming lessons.

Understanding "Give-It-A-Go"

I understand that Determined2 PTY LTD, any Medical Support or PADI (Professional Association of Diving Instructors) professional involved with Give-It-A-Go Does Not Teach, Certify or Imply that OPEN WATER DIVING is either recommended or supported as part of the Give-It-A-Go program or for me personally. I, understand That participation in Give-It-A-Go does not Teach, Certify or Imply OPEN WATER DIVING and does not allow me to SCUBA dive or use equipment outside of the Give-It-A-Go program. I, understand any medical clearances given to me is only for use within Give-It-A-Go, guidelines set out by the Medical clearance and those set by Determined2 PTY LTD and any Training staff within Determined2. If you decide that you would like to pursue recreational open water SCUBA diving, you will need to source independent advice, complete the appropriate medical clearances and undergo any Professional training within the set guidelines of PADI. I understand that I am not Medically cleared or certified to undertake any compressed Gas (SCUBA) activities outside of the Give-It-A-Go program, and agree to all the statements above.

Give-It-A-Go Statement of Risk & Liability (Non-Agency) Acknowledgement

I understand and agree that PADI (Professional Association of Diving Instructors) Members ("Members"), including Determined2 PTY LTD any individual PADI Instructors and Divemasters associated with the programme in which I am participating, are licensed to use various PADI Trademarks and to conduct PADI training, but are not agents, employees or franchisees of PADI EMEA Ltd., PADI Americas, Inc., or its parent, subsidiary and affiliated corporations ("PADI"). I further understand that Member business activities are independent, and are neither owned nor operated by PADI, and that while PADI establishes the standards for PADI diver training programs, it is not responsible for, nor does it have the right to control, the operation of the Members' business activities and the day-to-day conduct of PADI programs and supervision of divers by the Members or their associated staff. Statement of Risk and Liability This is a statement in which you are informed of the risks of Give-It-A-Go. The statement also sets out the circumstances in which you participate in the Give-It-A-Go programme at your own risk. Your signature on this statement is required as proof that you have received and read this statement. It is important that you read the contents of this statement before signing it. If you do not understand anything contained in this statement, please discuss it with your instructor. If you are a minor, this form must also be signed by a parent or guardian. Warning - Give-It-A-Go has inherent risks which may result in serious injury or death. Breathing compressed air involves certain inherent risks; decompression sickness, embolism or other hyperbaric injury can occur that require treatment in a recompression chamber. Give-It-A-Go can be a physically strenuous activity and you may be exerting yourself during this programme. You must advise truthfully and fully inform any Medical professional, program professionals and the facility through which this programme is offered of your medical history. I understand that the Give-It-A-Go program has been developed and is used by Determined2 PTY LTD and not PADI.

Acceptance of Risk

I understand and agree that neither the dive professionals conducting this program, nor the facility through which this programme is conducted, Determined2 PTY LTD nor PADI EMEA Ltd., nor PADI Americas, Inc. nor their affiliate or subsidiary corporations, nor any of their respective employees, officers, agents, contractors or assigns accept any responsibility for any death, injury or other loss suffered by me to the extent that it results from my own conduct or any matter or condition under my control that amounts to my own contributory negligence. In the absence of any negligence or other breach of duty by the dive professionals conducting this programme, the facility through which this programme is offered, my participation in this Give-It-A-Go program is entirely at my own risk. I HAVE FULLY INFORMED MYSELF OF THE CONTENTS OF THIS NON-AGENCY DISCLOSURE AND ACKNOWLEDGMENT AGREEMENT AND STATEMENT OF RISK AND LIABILITY BY READING BOTH BEFORE SIGNING THESE STATEMENTS.
